Household Vaccine Due Date Tracker
A one-page log for every vaccine each pet in your household has received, with the next due date, who requires it, and the reminder you set for it.
Reviewed for 2026, updated September 8, 2026.
Most lapsed vaccinations in a household trace back to dates living in three places: a clinic printout, a photo of a receipt, and someone's memory. This worksheet gives every vaccine for every pet a single line, with the date given, the clinic and lot number from the certificate, the next due date, and the reason it matters, whether that is state law, your boarder's policy, or your veterinarian's recommendation. Fill it in once from your clinic's full history, then update the line the same day any vaccine is given.
The example rows show the vaccines that appear most often on US dog and cat records, but your own list may be shorter or longer depending on where you live and how your pet spends its time. Intervals in the example rows are typical label ranges, not a schedule; the date your veterinarian writes on the record is the one that counts. If you want the arithmetic done for you, our annual pet vaccine budget planner on the tools page estimates the yearly cost, and the guide on this site explains how the puppy and kitten series and the adult boosters fit together.
